Do you take insurance?

• No, but I will provide you with a superbill, which you can submit to your insurance company for reimbursement.

What is a Superbill?

• A Superbill is used by healthcare providers as a primary source of data for creating a claim.  It is an itemized list of services provided to a patient, which is then submitted to payers (insurance companies) for reimbursement.

Will my insurance company reimburse me?

• Your insurance may consider me an ‘Out-of-Network Provider’ and reimburse you in accordance with your health insurance plan. I can provide you with a monthly billing statement (Superbill)  for reimbursement if you wish to submit this to your insurance company. Please check your insurance company’s policy if you plan to use this method, as requirements vary by company. 

I have medicare/medicaid, but I am willing to pay out of pocket.  Is this an option?
